EPA Fresh Checks Community Agency Partner Application

Help put fresh, local and healthy produce back within reach. Your agency may be eligible to distribute free EPA Fresh Checks -- $5 gift certificates that can be redeemed for fresh fruits and vegetables at the EPA Community Farmers' Market. Collective Roots is providing EPA Fresh Checks to Community Agency Partners that demonstrate that they can quickly and effectively distribute Fresh Checks to individuals and families that need assistance in purchasing healthy food, and that will redeem them at the market.

To become a Community Agency Partner and distribute free EPA Fresh Checks to your clients, simply complete the form that follows below.
